Team Application

 

Once you have registered online for an access account (see Getting Started) as a Team Coach, College Rep, Supervisor, etc…, the IDA Team Application form becomes available. You may access the team application from your sign up confirmation page or by clicking on “Team Application” under the “Join the IDA” button on your navigation menu.

 

When you click on the Team Application button or link, the IDA Team Membership Application appears. NOTE: The Team Application MUST be completed before individual members may join the IDA for your team.

 

The first part of the Team Application consists of your IDA Membership Type and School Information. Fill in the form with the requested information. All fields with the exception of School Address Continued are required. So, don’t skip anything.  Once you have completed the page, click on the “NEXT” button. REMEMBER, you can always go back and change or update information at any time.

 

 

 

When you click on “NEXT,” you are brought to the part of the application process that requires you to complete the Team Management including the Team Coach, the Official College Representative, the Team Coach Supervisor, and the College Rep Supervisor.

 

The first part of the IDA Team Membership Application Continued reflects all the information that you just submitted online. If you notice an error at this point, you may click on the “Edit School” button on the top of the page to make changes. That button simply brings you back to the first page of the application process with all the previously submitted information.

 

Moving on, the first position that needs to be completed on the application is Team Coach. NOTE: The processes for completing all 4 positions are exactly the same. You will notice a form that says “Find Team Coach.”  Enter the first name, last name or both into the form and click on “Find.” This will search the database for this person’s information so you do not have to type in all their contact info if they are already in the system. NOTE: You may enter in a partial name and it will present a list of people that sort of represent that name.

 

 

Upon entering my Team Coach’s Name (i.e. Tweety) and clicking on the “Find” button, a list appears showing me all the users with the first name of “Tweety.” In this example, only one “Tweety” exists. But, the list may be longer depending on the name. It should be noted that if there are no matches in the system, a message of “no match found” will appear along with an “Add New” button.

 

From this point, I have two choices. I may:

 

  1. Click on the “Select” button next to the name of my Coach.

 

OR

 

  1. Click on the “Add New” button to get a blank Team Coach Information form.

 

NOTE: Both the Team Coach Forms are the same.  However, if you get your Team Coach’s information by clicking on “Select,” your form will be pre-filled with the information that exists in the system.

 

 

 

 

 

 

When the detail form for the Team Coach appears, you can still view the previously submitted school information. Then, there are some detailed directions for completing the form. The first part of the form reflects the first and last name of the coach as well as the email address. NOTE: Again, most of the fields are required. So, it is wise not to skip anything.

 

 

 

The next section contains address and phone number information.

 

 

 

Followed by, USDF Information and Team Coach specific information including Employment Type with the school as well as licensing and insurance policy information.

 

 

The last question on this page asks whether or not this person is the Official College Representative. If you click yes here, acknowledging that this person is also the College Rep, the system will skip the college rep sign up process and submit the above information as the College Rep as well.

 

Finally, you have 3 buttons to choose from. If you click on “PREVIOUS,” you will be brought back to the first page of the Team Application with the School Information. If you click on “Add Another Team Coach,” the system will bring you back to the “Team Coach Find” form and give you the opportunity to add another person for this position. And, the “NEXT” button will continue the application process. NOTE: You will be able to add and or remove people from all positions at any time.

 

Upon successful submission, you will see your School Information and just below that the beginning of your “Team Management.”  In our example, “Tweety” fills the role of Team Coach and College Rep. You will notice that next to the Name for each position are an edit feature and a remove feature. Edit will bring you back to the form detail for that person. While remove will present an “Are you sure?” message to permanently remove this person from the Team Management.

 

The next required position is “Team Coach Supervisor.”  NOTE: The system will allow you to select yourself as the Team Coach Supervisor in addition to the other positions. In other words, one person could technically hold all four positions. Please recognize that if you do not work directly for the college, the IDA would like to have a contact person from the school in the supervisor positions.

 

After the Team Coach Supervisor information has been completed, the College Rep Supervisor position needs to be completed. Again, it is the same procedure.

 

Once all four positions have at least one person assigned to them, the Team Management portion of the form adds the ability to “Add New Coach,” “Add New College Rep,” Add New Team Coach Supervisor” and “Add New College Rep Supervisor.” So, you may add to these positions at any time.

 

Note: If you are registering as a supporting school, the next section will not appear for you. So, you may skip the next paragraph.

 

NEXT, FOR PARTICIPATING SCHOOLS ONLY. As part of your Team Membership Application, one member of the Team Management will receive a USDF IM Membership included free of charge.  You must check the box next to the person who will receive this IM Membership in the second column. NOTE: If you select more than one check box in the second column, the first name will receive the membership. The rest will be assessed the $25 fee. The next column asks if each person wants the IM Membership. Again, only one will receive the membership free of charge. The rest will be assessed a $25 fee. So, you must select Yes or No next to each person. The final column asks where you want your USDF Magazine Sent. This is only relevant if you selected yes in the previous column. When you have completed that form, click on the “Submit USDF Members” button.

 

 

 

 

 

 

Now, your application process is complete and the “Print Application” button now appears on the page – see below. Also note, the USDF info that you just submitted appears at the bottom (again for Participating Schools only).

 

 

When you click on the “Print Team Application” button, a printable form appears. You will notice that on the top right of the form, you may Edit and Print from that point. You must review this form for accuracy, print, sign and send to your Regional Representative accompanied with a check for your dues.  Once you have completed the form and printed it (again, you may re-print this form at any time), you may click on the “IDA Homepage” link on the top right to return to the main page.

 

 

When you do this, you will notice that your team name appears in your navigation menu just under the “Home” Button. This button will appear for anyone holding the position of Coach, College Rep, Coach Supervisor or College Rep Supervisor. From that point, you can manage your team, edit your application, add to your roster, print your Eligibility Certificate, etc… See the School Management section for more info.
